I think they are treating him for DBS (Deep Brain Stimulation). They check their reflexes and such while they stick electrodes into your brain while adding electric current. They adjust it on how you react. Apparently this guys is tuning himself with a banjo.

i dont think the hospital would let someone do that out of saftey reasons. But maybe if he has brain cancer wanted to make him happy. The noise would also interfere with the medics talking

It is required for the patient to be awake and talking durning a brain surgery so the doctors know they are doing it right. If the patient suddenly starts acting weird of having trouble speaking they know they did someything wrong with the brain.
